Overview: The purpose of this program is to support nonprofit organizations, area health education centers, and other nonprofit health organizations in North Carolina and South Carolina in their efforts to improve the health of Carolinians. Current funding priorities for this program are as follows:

  • Access to care for vulnerable populations: support for projects that expand access to health care for underserved populations, such as racial minorities and rural residents
  • Maternal and infant health: support for evidence-based programs and models that prioritize reducing maternal and infant health disparities and improving maternal and infant health outcomes
  • Mental health: support for models of care that increase access to quality mental health services and prevention programs, with a special emphasis on pediatric mental health care
  • Oral health: broadening access to oral care and reducing oral health disparities in children
  • Population health: support for regional community coalitions to help address upstream drivers of health
  • Workforce development: support for a community-based workforce that serves as a liaison between health care, social services, and the community, with the aim of improving population health and reducing disparities in health outcomes

Eligibility is limited to organizations located in North Carolina or South Carolina.
